Out of hospital care multi-disciplinary teams

This case study explores a team that was established with the goal of achieving a two-hour turnaround from identifying patients that can be sent home with support, to them arriving at home.
Key outcomes included:
- Achieving greater collaborative working with social care providers
- Improved understanding of roles, commissioning, referrals to services, gaps in services and effective ways of working
- Ensuring all patient needs are met in the correct environment, with the right person(s) providing the supportive care needed
